Parameters of ambulatory blood pressure monitoring and left ventricular hypertrophy in treated hyper

Objectives: High arterial blood pressure is frequent associated with target organ damage, including left ventricular hypertrophy (LVH). To better investigate of hypertension, we performed 24-h ambulatory blood pressure monitoring (ABPM) and evaluated the relationship between ABPM parameters and LVH. Methods: There were 1230 medical outpatient visit in our Clinic because of hypertension last year. The outpatients were included in our study by the following: first appearance, therapy-resistant essential hypertension, antihypertensive treatment was unchanged during the last three months, full examination, included echocardiography (Philips, iE33), and ABPM (validated instrument: Meditech ABPM-04). We checked with the Pearson correlation the relationship between the left ventricular mass index (LVMi) and the measured (24-hour, daytime and nighttime BP means), and from these calculated values (pulse pressure in any time, diurnal index, high BP time elevation index, high BP load, BP volatility and ambulatory arterial stiffness index) of ABPM. Results: The number of patients: 77, the mean age: 51 years (18-86 years), the body mass index: 28.14 kg/m², the LVMi: 152.09 g/m², the 24-hour BP mean: 138.07/78.31 mmHg, the daytime BP mean: 142.93/82.29 mmHg and the nighttime BP mean: 127.28/69.90 mmHg. The correlation between the range of the LVMi and the measured values of ABPM was only moderate, and significantly was just by 24-hour systolic BP mean (0.408, p<0.01), and by nighttime systolic BP mean (0.391, p<0.01). Although the correlation between the LVMi and the quartiles of increased ambulatory arterial stiffness index and the decreased volatility were stronger (p<0.001, and p<0.005). Conclusion: Present study indicates that in treated middle-aged essential hypertensive patients the 24-hour systolic BP mean, the nighttime systolic BP mean, BP volatility and ambulatory arterial stiffness index is associated with the LVMi. The correlation is not so close, therefore many other factors should be mind.
