Prof. Dr. Hugo Katus was born on November 30, 195 1in Steinfeld, Germany. He was a student of medicine at the University of Heidelberg and took a degree of MD in 1976. He is currently a distinguished professor and head of Internal and Cardiovascular Medicine at the University of Heidelberg. He was professor of Internal Medicine and Cardiology at the University of Lübeck between 1996 and 2002. After relocating to Heidelberg he was elected professor of Internal Medicine as well as president of the Academy of the German Society of Cardiology. Professor Katus has been voted speaker for the Cardiovascular Disease Section of the National Genom Research Initiative. As an editor of the German scientific journal he has been active since 2004. The wide research interests of Professor Katus include the fields of experimental electrophysiology, clinical hemodynamics, biochemistry, immunology, molecular biology, as well as clinical genetics and clinical chemistry. The groundbreaking diagnostic method for the detection of myocardial cell necrosis (troponine) is also related to his name. The outstanding publication achievements of Professor Katus include about 500 original articles and book chapters. The accomplishment of his contribution to the advancement of cardiovascular science has been recognized with several prominent awards.

Abstract

The usefulness of cardiac troponins for detection of myocardial cell necrosis and risk stratification has been established beyond doubt. Cardiac troponin testing is a key diagnostic element for the diagnosis and management of patients with acute coronary syndromes without ST segment elevation and is increasingly used in non-coronary diseases to indicate prognostically important cardiac damage. Given the biochemical and analytical differences of cTnT and cTnI there is ongoing controversy regarding the comparability and clinical performance of cTnT and cTnI. cTnT and cTnI are both expressed in cardiomyocytes but differ with respect to biochemical and analytical characteristics. While minor differences of analytical precision or biochemical properties are not relevant for diagnosis and management of patients with acute coronary syndromes and most diseases with non-coronary related elevations of cardiac troponins, these differences may be amplified in patients with chronic renal failure. In fact, recent studies in patients with end-stage renal disease under chronic hemodialysis have readdressed the issue whether cTnT and cTnI are really comparable. The present review will provide a state-of-the-art overview on the performance of cardiac troponins in acute coronary disease and other clinical conditions.
